The negative outcome would have been horrific.
Its bad enough when two aircraft strike one another like in the case of KLM Pan Am Tenerife Accident (1977), the worse aviation disaster in history:

If the Air Canada pilots didn't start their go around when they did they probably would have taken out 2 other aircraft at a minimum.
Thats one very bad day at work for those 2 pilots.
Fairbssays...I don't claim to know much about this, but in the audio it did sound like they were seeking clarification about the runway being on the right; I'd be curious to know their airspeed at the time of the avoided colision; would a plane coming down on top of another crush the one underneath? If they hit nose to nose it definitely would be ugly

Fantomassays...They were, for the runway on their right, which is not Runway 28R (the thicker red line).
Edit: The thick line below that is 28L, I'm not sure what the visibility was like, but to confuse a taxiway for a runway is pretty incompetent.
That's a drastic error on the part of the pilots (although I can see how it could cause confusion).

Jinxsays...Don't big airports usually have a device that pretty much lines the landing up for the pilots? Will be interesting to hear exactly how this happened.
