Is there an appropriate etiquette that wasn't being observed or is trail riding just an exercise in waiting for the fatal moment?

Standard, legal trail? Yes. Etiquette was ignored by one or both. ie. Keep right on corners and/or one direction only.

Standard, illegal trail? No Man's Land. ie OH FUCK!! WAS THAT A COP!!?!!? GUN IT!!!

fake? he just happened to have a camera on his helmet?

I have never heard of a head-on motorbike collision. To this moment I still have not.
