thats mad lol , the bit where the boxes fall down and lift the car up you could so easily replace those boxes with an approximated mesh with a good water shader and you have just achieved the coolest water affect in gaming history.

From his Youtube description, this isn't actually real-time rendering. In fact, in the video he notes that each frame took 8 seconds to render. It looks awesome, but there is no way you're going to get the CryEngine to actually do this for you at 30fps. He actually made the video by taking screen captures of each frame and then compiling them into a video. This had to be incredibly time consuming.

As Lurch writes, this isn't real-time. At -3:34 it says

"sys_physics_cpu 0 significantly reduces framerates, that was rendered at around 8spf; 8 seconds per frame!
Remember to also use fixed_time_frame
I used 0.33333 for both clips."

It's still awesomely cool, but not real-time.
