It should be easily fixable, I'd think.
Ya, It's like one one in ten goes wonky and then it just can't be fixed not matter what you do...
The first time it happened was 6 months and 1 week ago actually...Exactly when the @usename feature was added, which is also responsible for the bug gwiz mentioned. Something was changed in the code that's responsible for these bugs.
I think there is, or has been, multiple bugs because I remember comments getting mangled starting the day we got the new quoting system. For some reason, a lot of them seem to 'work themselves out' after a couple hours.

Two other bugs that I encounter ever so often.
-Up vote button waits eternally and never casts my vote.
-Sometimes hitting the comments button from one video takes me to the comment section of a previously opened video on the same page.
I get the same "malformed submission" error when trying to upvote a video, or the vote hangs on eternally until I reload the page and try voting again.
Clicking 'profile reply' and the pasting that into your desired comment location seems like a good temporary work-around.

when that happens, you've been logged out - sort of. You can see the "New User" where your name should be. I think a fix for that is to reload the page, which gets you properly logged in, and then logging out and in again... then the cookie is going to be refreshed for a while and it won't screw around. Worked for me, anyway.
