This has happened a few times before, though it is this video that inspired this post. A custom thumbnail needs to be an actual still from the video. This was established by Lucky in the post in which custom thumbnails were announced (thanks for pointing that out, Burdturgler).

Perhaps there should be a function to flag inappropriate thumbnails?
