I wonder about the deaths just below the club, but not on the ground. In the Roaches mission where you go to the club, the final part is you have to detach a bomb from the bottom of the club. If you fall out with the bomb, it might go off with you next to it, just under the club. I wonder if those are those deaths. It says it is impact deaths though, so...?
