that's an easy one... He has a string in his mouth that holds the cigarette.
If you look at the way his hands move it doesn't make a true rotation around the cigarette. That, and his head's always directly above it.
Sorry, I hate to be a spoil sport but I love illusions and this one is a fun but simple one.

How could he continue to raise the cigarette with he mouth open? Also, while his hands don't do a rotation, he does pass a hand completely OVER the top of the cigarette.

Obviously someone off camera is manipulating the cigarette. Also notice when he put his hands in a circle to show there were no strings he broke the connection of his hands.

There is an "invisible" thread attached with wax on the cigarette and the other end on the right thumb with the middle of the thread attached to the head.
