I had something similar in the early 80s from the inside of MAD Magazine - only the song was Wierd Al Yankovic singing "Another One Rides The Bus"

It was a page made of paper thin vinyl which was stitched into the seam of the magazine and you could tear it out and place it on an actual record player.
