It looks like they're draining an abscess, from how much blood is coming out. The clear fluid is the infection, and from the lack of puss, looks like this isn't the first time they're done this to that infection point. Then again, this is infection knowledge is bases off of doing the same thing to cats and dogs. You'd be surprised at how much puss a tom's face can hold after a battle with the local alpha male.

A quick google search suggests they're draining an infection in one of the teets. The infection can kill a cow pretty quickly if drastic measures aren't taken.

The lady handled it all in stride w/ a "what? it has to be done" attitude untilllllll......
Then, she screamed.

It just kept on coming... The horror....

So that's what they used in the blood-filled elevator/hallway scene from The Shining...
