This is not overtly sexual to me. It could be, if it was your kind of thing, I guess, but to me, it's simply about disturbing the viewer, which I find distasteful, since it's at the expense of two unhealthily overweight people.

It's certainly not the worst kind of image to be found on the internet, with this intent, but it's not something I want to endorse by keeping it in my channel.

I've never been this overweight, to know what it feels like to be these people, so I can't speak for them. Maybe my empathy is misplaced, but I feel that by deliberately being made a source of shock/horror, these people and their filmers and the disturbed viewers, who can't help but stare and gasp, do nothing toward engendering feelings of respect for all people, no matter what their physical appearance is.
