I want this to be real but my instinct screams fake. The sound quality is a problem. If they just had hidden mics then there would be a lot of ambient noise but they're so clear it's like they're wearing lapel mics. What do you think?

Derren Brown has made a point throughout his career to not do anything fake. Hypnosis, misdirection, psychology and illusions that are the basis for all of his tricks.
I strongly recommend you check out a few of his other shows.
