When a water bomber almost flies into a mountain at a fire

Ok, this is super nerdy but for the folks on Wings, it might be interesting. It's the interagency review of a near-accident.

CFIT=controlled flight into terrain.

This post-hash review talks about how the accident review highlighted how both pilots got Target Fixation (laying down the fire retardant line) and entered a state of task fixation (losing tracking of all other jobs).

The PIC (pilot in command) and his co-pilot missed that they descended below the level of the "hard deck": below which they should never have gone below---watch the wake turbulence ruffle the brush on that hill.
