Bionic Hand
Speaking as the parent of a child with a congenital amputation, based on the many hundreds of amputees I have met at conferences I think it is fair to say that there are many causes of amputation, and stupidity is rarely one them. Among leading the candidates (*not* in order frequency or anything):
1. Congenital amputation
2. Diseases such as a variety of cancers and meningitis
3. Accidents (work place and otherwise)
4. War
There seem to be a lot advances in upper-limb prosthetics just around the corner. I'm sure many of them will never pan out for broader use for one reason or another, but I hope a least a few of these technologies live up to their promise.

ReWalk: turning paraplegics into robocops
Pretty cool stuff! There are a number of prosthetic arms that utilize the same sort of chest-muscle sensors for control coming onto the market, as well.
If you were curious about the intro note, this man has 'SCI T8', a spinal cord injury at the level of his 8th vertebrae (which actually results in a 'T12' neurological pathology, where legs and bowels are no longer in controllable communique with the brain).

Sorry to pop the bubble here, but as a "World First" this has already been done, as far back as 2000 [Dr. William Dobelle]. There are many institutions working on this approach to making a prosthetic ocular device.
This one's approach is called a Retinal Implant, but the Dobelle approach (and many others) use a direct Neural Implant (complete with head-socket). They tested the implants/sockets on 9 individuals since the early 1980's and found little to no infection or contra-indications. It afforded a vision of 20/200 in the initial trials and improved up to 20/80 in the later trials. This later approach requires an external camera (likely mounted to a pair of glasses). I believe you can still get this operation done in Portugal.
The former approach of Retinal Implant (JUST like they show here) has been functioning by the Argus Group (USC & John Hopkins) since 2002 and is already approved by the FDA for a second generation trial of 60 individuals.
What puzzles me is why we hear so little about these amazing breakthroughs (years after the fact) and are instead so "up" on 'popular' news to the minute.

Transplanting the Human Head
Don't forget that we have devices to artificially monitor and maintain a heart beat, devices to breath for you, devices to filter your blood (to an extent), etc.. That said, to keep a head alive on a donor body I'd imagine they'd focus on reconnecting innervation to the new heart, innervation to the new diaphragm, and potentially to a large muscle group that could then be tied through external sensors to operating basic prosthetics. The person would probably live like a C1/C2 quadriplegic, breathing and speaking with the help of a ventilator, using a pacemaker and other devices to monitor and maintain the body. He would also be taking intense immune system inhibitors, since his head would be a giant pile of antigens as far as the body's blood was concerned.
