WTF, were these people confused about how many teeth were to be extracted?? Usually you have some idea when you show up to do an extraction... if the dentist is trying to take ALL of your teeth out, STAND UP AND LEAVE.... seriously... did he have these people strapped down? They don't usually put you out for extractions. You'll KNOW if they're taking out extra teeth..

This is partly because such studies are hard, and rare. The most commonly cited one (which is unusual, as you point out) is the Nurses Study.
An interesting one mentioned by the TV show "Supersizers go..." was studies into the effects of wartime rationing in WWII London (the first episode of the series actually).

As they point out, although certain details change, the basic advice has stayed the same for the past 50 years... eat less sugar and fat, eat less meat, eat more fruit and vegetables. No matter which trend is in at the moment, eating too much processed food is always discouraged and, with few exceptions, eating more fruit and vegetables is usually encouraged.
Re-watching the end of the episode though, reminds me that it wasn't actually the last few minutes where they mentioned the research, it was earlier. Rats. Somewhere in the episode they did make mention of it, I'm sure of it, but I only found the summary at the end.
In his short and readable book "In defence of food", Michael Pollan also mentions research by Canadian dentist (!) Weston Price, published in 1939 and titled "Nutrition and Physical Degeneration". Price studied the diets of various isolated populations that had not been exposed to modern food, and found that humans are quite adaptable and can thrive on a wide variety of diets... sadly, the Western diet does not appear to be one of them.
In the same volume he also points out that you should avoid foods which make health claims... if you look in the supermarket, the healthiest foods are likely to be lumped together in the fresh fruit and vegetable section making no claims at all. If you want a book which provides references and studies, that one is worth reading. I'm sure he is not 100% correct in every claim he makes, but like I said, the book is short and readable.
One of the things that really annoys me about debates on nutrition, is that there are almost never any actual studies cited. Tons of anecdotes, but anecdotes are not evidence.
Frankly, I've had to tune them all out in self-defense, or I'd be switching my lifestyle on a yearly basis to go along with the latest fad.

American Dentistry is a huge racket. A single dental implant, (not covered), mysteriously cost right around $1700 USD. The price is the same at any dentist you go to in the entire country.

I've lived in Thailand for almost 6 years now, and I was a poor college student/graduate prior to that. So, I hadn't had any dental treatment whatsoever for about 10 years. At that time in the US in my small hometown, I figured that a cleaning with nothing else done would cost $100. Plus the dentists wanted to do x-rays every year that would be another $50-$100. So, basically $150-200 yearly assuming that I didn't get anything other than a simple cleaning.
So far I've been lucky to have teeth that stay in pretty good condition. I hadn't had any cavities in adult teeth, but 10 years without a trip to the dentist was pushing it. So, I figured it was time to bite the bullet and visit a dentist here in (upcountry) Thailand. I walked in and they said that I had 3 cavities and 2 small weak spots that could turn into cavities.
They asked me if I would like old-style metal colored fillings or new tooth-colored stuff (I didn't even know that existed). I opted for the old style since it was slightly cheaper, but the doc was upset about the prospect of putting the first visible fillings in my otherwise pristine teeth so she said she would give me the tooth-colored ones for the same price as the other type.
In and out, far less pain and discomfort than I remember being standard at my dental visits in the US (to be fair I had a lot of cavities and problems with my baby teeth when I was a kid, plus braces and lots of orthodontic work). Total cost -- about $40 USD. For yearly cleanings, I think the local clinic here will charge about $15 USD. So, I'm well pleased with the results.

Yes, I heard about this when Marketplace released it.
But let's be honest. No one goes to Dentists they've never been to before out of the blue for random surgery. What they suggest is purely a suggestion. Like any other business, the up-sell for cosmetic reasoning will always be available and just because they offer it, doesn't mean you have to do it.
The prices they quote are also the price before or without dental coverage. (I do understand not all coverage is the same)
There is also going to be overzealous Dentists, usually new dentists who just want to do all those big procedures because they enjoy the bigger jobs.
No one should be rushing into these procedures. You should stay with one dentist over time and build a relationship with them. I've never had a issue with any of my dentists. I've heard of people being offered specific services but they are all elective - and few move forward with them unless necessary.
I avoid any dental work for my family or myself if it's purely for cosmetic reasons (especially whitening- teeth aren't supposed to be white). You don't want to do it? Don't do it. But they are going to let you know your options. I think this is different than being told, "You must do it, if you don't, all your teeth are going to fall out and you will be in pain for the rest of your life."

For me, and this may not be the exact definition of productivity - but productivity is the cost of getting shit done.
Anyone who has been to the dentist, tried to re-roof their house or start a small business would probably agree that it's pretty expensive to get stuff done here.
Which isn't to say that I don't like how things work here. I like big public services and don't begrudge the high taxes. I like that there is a huge middle class here.
But ... And this is the part that's going to get me some flack, there is a certain softness that comes with a 5% unemployment rate. We feel very entitled to the jobs we have - because there is no one else waiting in the wings to do them for us. Employees have a huge leverage stick here in Canberra because there is literally no one else willing or available to fill the roles.
Lastly, I want to make it clear that I'm not defending the awful views of this lady.

this cat i was watching after it got treated at the vet, had a victorian collar, and passed out from the medication with its head in the bowl.... i thought it was still eating so i petted it and it woke up and ran all around like a maniac until it passed out again drooling... i wiped that drool and talked it through the trip... it wouldnt blink for hours, i had to pet its nose to make it blink.
