Over 99.9% of atoms heavier than Helium-4 were formed via nuclear fusion inside stars. Other natural sources include spontaneous fission, big bang nucleosynthesis, cosmic ray spallation, radioactive decay, photodisintigration, neutron/alpha absorption, and (theoretically, I may have just made this up) ejection of material from neutron stars.
